OUR STORY

Dr. Coretta D. Keith-Bryant, founder and owner of CDG HEALTHCARE PROVIDER, P.C., a.k.a CDG Healthcare Provider P.C., which was established in 2014. We are a local, independent, licensed private home care provider serving the needs of the residents of Southwest Georgia. Our passion is committed to providing the best client-centered care right at home. We also provide dedicated service to our clients, building relationships with high-quality care. Our caregivers are compassionate, caring, energetic, and responsible.

CDG HEALTHCARE PROVIDER, P.C. provides NURSING, PERSONAL CARE, and COMPANION or SITTER services to individuals who need assistance with their activities of daily living to live independently in their own homes. Services are delivered by qualified personnel and are sensitive to the unique needs of the individual and the community. The task to be performed are provided by trained, qualified workers and provide it in accordance with the individual service plan and state rules and regulations, as well as the agency’s policies and procedures. Services are supervised by managers who are qualified based on education, training, and experience to provide such tasks in accordance with acceptable standards of care.

This private home care agency was founded based on the owner’s personal experience growing up with a special needs sister, who was placed in multiple institutions off and on throughout her life, to provide the unique care tailored to her needs that was not available in the home. As an adult, the owner also experiences caring for her elderly mom, who is reluctant to live anywhere else besides her home. She requires assistance with her activities of daily living, light housekeeping, meal preparation, medication administration, grocery shopping, and transportation to and from doctors’ appointments. One day, we may all share a personal experiences of being the family caregiver for our own loved ones. We will experience many challenges on a daily basis.

Before embarking on a journey toward private home care, Dr. Coretta Delia Keith-Bryant has a combined nursing experience of more than 25 years. After graduating from Georgia Southwestern State University in 1995 with an Associate Degree in Nursing and passing her state board exam, she worked as a registered nurse in the emergency department for Phoebe Putney Memorial Hospital. While working as a registered nurse, she continued to educate herself by obtaining her Bachelor’s Degree in Nursing from Georgia Southwestern State University in 1997.

She continued to advance her knowledge and enrolled at Albany State University and received her Master of Science in Nursing in 2001 and became a certified family nurse practitioner. As a certified nurse practitioner, Dr. Coretta gained experience from working in multiple emergency departments, such as Sumter Regional Medical Center in Americus, GA., Grady Memorial Hospital and Emory University Hospital both in Atlanta, GA. She also worked as a nurse practitioner in South Georgia Medical Center-emergency Department and currently Tift Regional Medical Center.

Her thirst for knowledge on evidence-based practices led her to further her education by obtaining her Doctor of Nursing Practice from Robert Morris University in Pittsburgh, Pennsylvania, in 2010. She continued to work as a provider in the Emergency Department but also served in multi-management roles as a chief advanced practice provider for numerous physician assistants and nurse practitioners. Dr. Coretta is certified in advanced cardiac life support, pediatric advanced life support, CPR, and basic life support. Dr. Coretta has a passion for caring and providing professional, empathetic patient care.
